Medicare works differently in Puerto Rico
Medicare on the island is its own world: most beneficiaries choose Medicare Advantage (the "Platino" market), Medigap is generally not sold, and Puerto Rico doesn't participate in HealthCare.gov. One more critical difference: unlike the states, Part B enrollment is NOT automatic for most Puerto Ricans receiving Social Security — you must actively sign up, and missing it triggers the same lifelong penalty.
