Short answer: Your window is 7 months around your 65th birthday — enroll in the first 3 months. Military family? TRICARE For Life requires Part A and Part B, and VA health care alone doesn't let you delay Part B penalty-free — the two most expensive misunderstandings in Virginia Medicare.
Virginia is home to one of the largest military and veteran communities in the country — from Hampton Roads to Northern Virginia — so the turning-65 conversation here often involves more than just Medicare. Here's the full picture.
Your 7-month Initial Enrollment Period
The window: 3 months before your birthday month, the month itself, and 3 months after. Enroll early for no-gap coverage; miss it without creditable coverage and the Part B penalty (10% per year, for life) and Part D penalty apply.
Military retirees: TRICARE For Life needs Part B
TRICARE For Life is excellent wraparound coverage — it pays after Medicare with little paperwork. But it only works if you're enrolled in both Part A and Part B. Decline Part B and you can lose TRICARE coverage entirely. For military retirees, the 65th birthday isn't optional paperwork — it's the linchpin of your whole benefits stack.
Veterans: VA care + Medicare are partners, not substitutes
VA health care covers you inside the VA system; Medicare covers civilian care everywhere else. Two crucial nuances:
- VA drug coverage IS creditable for Part D — many veterans skip a separate drug plan safely.
- VA health care is NOT creditable for Part B — relying on VA alone and skipping Part B triggers the lifelong penalty if you enroll later. This catches veterans every year.
Many Virginia veterans carry both VA care and Medicare for maximum flexibility — and the PACT Act expanded who qualifies for VA care in the first place (see our PACT Act guide).
