Medicare runs on a calendar. These are the three enrollment windows everyone should have circled.

Initial Enrollment Period

Around your 65th birthday

3 months before to 3 months after your 65th birthday — your first chance to enroll in Medicare.

Annual Open Enrollment

October 15 – December 7

Each year, you can switch, drop, or join Medicare Advantage and Part D plans for the year ahead.

What is Medicare Part D?
Part D is Medicare's prescription drug coverage from private plans approved by Medicare. You can get it as a standalone plan alongside Original Medicare (or a Medigap plan), or built into most Medicare Advantage plans. Each plan has a formulary — its list of covered drugs organized into cost tiers.
How much does Part D cost, and what is the new out-of-pocket cap?
You pay a monthly premium plus copays or coinsurance for your drugs. As of 2025, Part D has a $2,000 annual cap on out-of-pocket costs for covered prescriptions — once you hit it, covered drugs cost you nothing the rest of the year. Higher earners also pay an income-related surcharge (IRMAA).
How do I choose the right Part D plan?
The lowest-premium plan is rarely the cheapest overall. What matters is your total annual cost — premium plus what you pay for your specific drugs on that plan's formulary and preferred pharmacies. When can I enroll in Part D, and is there a penalty for waiting?
You can first enroll when new to Medicare, then change plans each year during the Annual Enrollment Period (October 15 to December 7). Go 63+ days without creditable drug coverage after you are eligible and you will owe a late-enrollment penalty added to your premium for life — so it usually pays to enroll on time.
